I have some information about chakras on my web site, but I'll see if I can give you a bit of help with your questions.
1. I have heard that it is important to keep my chakras balanced. How would I know if they were not?
It is important to keep them balanced. The easiest way to know that one or more of your chakras are not in tune is the appearance of health problems in an area associated with a particular chakra. Let me give you an example from my personal experience.
On of the biggest problems I used to have was that I would let people take advantage of me, but I would never stand up for myself or vent my anger away. I would be furious about what someone had done to me, but they would have no idea that I was so angry I thought I was going to self destruct. I just kept all that anger pent up inside. Fast forward a few years and I'm seeing a specialist for my fibromyalgia and within less than 5 minutes, she looks at my neck and says I have a problem with my thyroid...

That is the location of the 5th chakra. The biggest trait is that it stands for communication. Because I was purposely NOT communicating so much, the problem expressed itself physically. The anger I was holding inside turned into non cancerous tumors on, and around the entire chakra. I started with 1 tumor, and ended up with 8!
2. If I discovered that my chakras weren't in prime condition, how would I go about remedying that problem?
There are several ways to go about keeping your chakras healthy. Meditation, yoga, sound therapy (making certain noises that causes particular vibrations). If you know you have a block in 1 or more chakras, I suggest you find out what the chakras govern (like communication, for example), then meditate and try to understand what you are doing to cause it to be blocked and go from there.
3. Are all of the chakra centers connected (if one is malfunctioning does that hinder the others)? Or, is each center autonomous from the others?
That's probably something people could debate, but my personal philosophy is that "everything is connected", so if one thing is out of whack, so to speak, it throws your whole body out of balance to some extent.
4. What are the short and long term effects to having unbalanced chakras?
That depends entirely on how serious the problem is, what is causing it, and a persons willingness to 'go all the way' and to whatever it takes to stop doing what ever is causing the probmem/s.
