I know a lot of people simply won't answer this because of any number of reasons - afraid of the response, not really sure what they would do, etc. But I'll tell you my opinion because you asked and because I'm never afraid to open my big mouth.
Because each case is different and everyone's situation is different, you are right - this is a difficult question to answer. There are any number of people who would say that you have already gone above and beyond to the EXTREME what most would consider noble in trying to save a life.
In my opinion (and it's JUST my opinion - based on my own ownership of cats....), when they get to the point where they are unable to get to the litterbox in time (and that's different than simply not caring about the box), they are embarrassed and their quality of life already has started to degrade. Especially in renal failure. It's painful and it's terminal. I would help them over the bridge.
Sometimes its hard to see the right thing to do because of our own feelings on the matter.
Big hugs to such a tough question. Again, each case is different....but it sounds like the decision I would probably make for Curie.....
