For any minor who wishes to enter the pits at the Mottville Speedway there are 3 seperate forms both parents must sign. There is a seperate form for the father to sign and the mother to sign and a 3rd form that both sign. All 3 must be notorized! This can not be done at the track! We can't email them to you but we can mail them or you can pick them up at the track on race day or at our office at 13774 N. State Road 19, Nappanee, In. 46550. To get the forms by mail call 574-215-4288 or email HOLDENM@NATIONWIDE.COM. The forms make it cyrstal clear that we are not responsible for any injuries to the minor!
