Not decided by insurance considerations. If the physician orders it, insurance will cover it. There are some tests that the medical profession finds to be meaningless under circumstances. For example, prostate cancer is so slow growing that at some point one can say the patIent will die of something else before the cancer does him in. Thus, why put the person through surgery,and/or radiation with the side effects? Neither the government (Medicare) or the insurance companies make the decision that some one is too old to treat.
